Google Forms:
Follow these steps to make a Google form:
Log in to your Google account > Documents
Select New on the Document task bar > Forms
Create the questions in your form
Select the Email This Form tab and enter your students' emails
Send them the form
Note1: It helps if your students have a gmail account, then you can put them in your contact list in gmail and email them using that list
Note2: The form will show up after you've created it as a spreadsheet, so you can go in and manage your students' responses
